Please print and fill out in BLACK ink the following patient forms and bring them with you to your first visit. If you have all of your paperwork filled out & with you, we ask that you be here 20 minutes prior to your appointment time so we can go over forms, give you any additional paperwork you may need to fill out, collect co-pays if applicable, and enter you into the computer system. If you do not have the forms, we ask that you arrive 30 minutes prior to your appointment time to fill out the necessary paperwork.
